THIS MEETING REQUEST IS FOR INFORMATIONAL AND BIDDER QUALIFICATION PURPOSES ONLY AND NOT FOR RECEIVING BIDS.  ALL PRIME CONTRACTORS WILL BE REQUIRED TO FILL OUT A “NEW” QUALIFICATION FORM EVEN IF YOU HAVE BEEN PREVIOUSLY QUALIFIED WITH Triangle Associates, Inc. AS THIS FORM IS PROJECT SPECIFIC. 

THIS DOCUMENT MUST BE FILLED OUT COMPLETELY AND RETURNED TO Triangle Associates, Inc. EITHER PRIOR TO SUBMISSION OF YOUR BID OR WITH YOUR BID.  THOSE CONTRACTORS/SUPPLIERS WHO DO NOT SUBMIT THIS FORM AHEAD OF THE BID RISK NOT BEING QUALIFIED FOR THIS PROJECT AND HAVING THEIR BID REJECTED.

PROPOSALS ARE DUE

All hand-delivered proposals are due on Wednesday, February 15, 2012 at 4 p.m. (the Due Date) at Jenison High School located at 2140 Bauer Road, Jenison, MI 49428 in the Media Center. Subcontractors using USPS are to send Bid Proposals to 8375 20th Avenue, Jenison, MI 49428. Subcontractors using FedEx and UPS are to send to 7506 Church Court, Jenison, MI 49428. All proposals received on or before the Due Date will be publicly opened and read aloud no later than 4:15 p.m. in the Jenison High School Media Center located at 2140 Bauer Road, Jenison, MI 49428.

PRE-BID MEETING

A pre-bid meeting for this bid package will be held on Wednesday, February 1, 2012 at 3:45 p.m. at Jenison High School Choir Room located at 2140 Bauer Road, Jenison, MI 49428. Please use the pool entry on the south end of the west parking lot. All bidders are encouraged to visit the jobsite.  Site examinations are available after the meeting.

Prevailing wages do apply to this project. 

Payment and Performance Bonds may be Required.

As required by State Law (P.A. 232 of 2004) all bids shall be accompanied by a sworn and notarized statement  disclosing any familial relationship that exists between the owner and/or any employee of the bidder, and any member of the Board or the Superintendent of the District. The Board shall not accept a bid that does not include this sworn and notarized disclosure statement.

A refundable deposit of $200.00/set is required and will only be returned upon receipt of bid and timely return of documents.  For a non-refundable deposit of $50.00/set Triangle will send these documents by US Mail or U.P.S.

Unsuccessful bidders are required to return bid documents to Triangle Associates within (30) days of bid due date.  Unsuccessful bidders will forfeit plan deposit if bid documents are not returned within allotted time.  Plan deposits will be returned to successful bidders within (30) days.

PROPOSALS ARE DUE
All proposals are due on Thursday, February 23, 2012 at 2 PM (the due date). All proposals received on or before the due date will be publicly opened and read aloud no later than 2:10 PM at Hudsonville Public School Administrative Building – Community Room, 3886 Van Buren Street, Hudsonville MI 49426.  Bidders need to park on Van Buren Street (due to the limited parking spaces at the Administration Building and DO NOT park on the grass).
